"On hitting" already implies a successful hit. Otherwise it would say "on swing" or "when attacking", etc.
All such "on hit" conditions are like this. The edge case is: if you hit, but DR reduces damage to zero, does this still count as a successful hit? I forget the answer, actually, but it's the same for all "on hit" effects.

Is it necessary to make a successful melee hit for inflicting a condition hit ??

Otherwise there are only conversations. There is no way for the player to give a condition just by talking with an npc, although you can receive a condition in a conversation, generally due to reaching a quest step (e.g. rotworms, the temporary blessings from the priest in Loneford, etc).

If you hit the target, the condition is applied, no matter the damage delt (even if 0).
That is also one of the reasons why block is better than dmg reduction.
Block reduces damage AND conditions by lowering the chance of infliction. If blocked, the dmg and condition are not applied at all.
Damage reduction lowers the dmg you get by x, but does have no effect on likelyhood of getting hit and therefore has no effect on conditions (even if 0).
Even if you have enough DR to nullify all dmg, applied conditions can stil kill you
