What does the Blackwater misery mean on it? I'm confused

It's a disease which reduces AC by 50%, but doesn't stack with other items that carry the same effect. Thus the penalty only affects you once.

my bw dagger says its and ap of 3 but when i equiped it i am stil at 4ap. Is this a bug or effect of.the.disease
