Noticed two duplicates of Torilo and cook spawning in the Foaming Flask tavern
This happened immediately after dying during combat and spawning inside the tavwrn

Yes. All of them could be interacted incl duplicates.
Currently, I have reloaded the save file to correct the bug/glitch. I'll try to fall unconscious in the tavern and respond back if it happens again.

Not us. We unfortunately have zero information on updates to locked devices. There are multiple companies who contract to the different institutions for the devices. Each likely has their own method of control/updates in order to stay in line with the various laws and local ordinances.
