I agree with your points. I also thought the text (and logo) needed recentering, but I'm just working with the image nyktos posted to the forums. I don't have the original file, which is presumably layered.

In landscape mode the spine could still be on the left. There were old books of art made like that.

I darkened the top, right, and bottom edges a little too (although it doesn't show very well in the image below; probably needs a little more):

im thinking of a background that's not black...
and working the image in to look more like a book standing out from the BG...

Sorry. I know that's a really crude hack job, but it's what I could do in 5 minutes with nothing more than a bitmap image to start from, and it's good enough for a general impression.

Edit: I made it look a little less horrible :D

All this looks great, but it doesn't look medieval. It looks like a book that was printed anywhere from 1940 to now. I thought we wanted a medieval look... didn't we? That's why I did the gilding in the first place. So how about medieval binding?

True. We are getting closer though. If we have the background, the logo, the font, and features like a spine, we have a basic book design. Then we have to make it look medieval. So gilding, some wear and tear, etc.

Maybe we need some clasps, and other metalwork. This is a real medieval book:
